I'm not sure what types of tests a gynaecologist would perform, but I'd think that while looking at her hormones and psycology there would be a bit of leeway as to what would be a pass or fail for genders. Men and women aren't uniform in their endocrine or mental make up, and there are bound to be some women who have the mentality of a man, and hormones out of the ranges of average ranges of women, and the same for men.

I think the easiest and most difinitive way to tell would be a DNA test using just an easily obtainable smear from the inside of the mouth. It wouldn't be the cheapest or the quickest, but its got to be better than a hormone and psycology test.

I always figured that really only a visual test would be needed for something like that. It ought to be easy enough to tell from that.

There were athletes in the past who earned a mark next to their names after death because during postmortem examinations doctors did discover that they were men. It is not that easy to just judge by appearances. There was one Polish athlete who did compete successfully before World War Two, and who during a postmortem examination was declared a man. No one in Poland, nor across the globe did notice anything ‘strange.’
